Intake Process
Before you submit an inquiry form please take a moment to learn about our qualification process, and how we’ll determine, together, whether HOPe is the right match for your family member.
When you submit the inquiry form, a few steps need to be reviewed and completed before we can welcome your family member to HOPe. Let us know if you are interested in HOPe’s programs and services and let’s get to know each other.
The HOPe Intake Process
Step 1: Inquiry Sent to HOPe
Once we receive your inquiry into our programs—either directly via email, phone, or through our online inquiry form, or through a referral packet from DDS—we’ll contact you to learn more about your child and your needs and answer any questions you may have.
Step 2: Complete Qualification Form
If you decide to go ahead with the qualification process, you’ll complete a HOPe Qualification Form. Your DDS Coordinator should also send over their referral packet if they haven’t already.
Step 3: Meet & Greet Scheduled by HOPe
HOPe staff will schedule a Meet & Greet with your family.
Step 4: Visit Day is Scheduled (if needed)
Your family member will be invited to for a Visit Day, in-person, remotely, or both.
Step 5: Intake Decision is Made
We’ll make a final decision on whether we can accept your family member for membership.
Step 6: Intake Packet Completed
If your family member is formally accepted into HOPe programs, we ask that you complete our Intake Packet (Releases, Medical Forms, etc). Once all forms are in, the applicant is cleared to attend services!
